She was not able to catch her breath, and she fell on the hospital floor. It was reasonable that Willow had some blood work done, but things didn’t go that far.
Viewers know that Harmony is doing everything she can to hide the secret of bio-mom from Willow and everyone else. The fake birth certificate was apparently good since Willow got her expedited passport without any issues. But, it seems like her lie might get caught soon.

The speculation is that Willow might be stressed because of Wiley’s court battle that is taking a toll on her. So, she might get hospitalized again. If the doctor orders any test, it might suggest that Willow has some serious problem, and she might need her biological parents to get back to normal.
When Willow will need her bio mom, viewers know that Harmony will be ruled out as an option. All of this might somehow lead to Nina stepping up for her real daughter and saving her life. The soaps always have some blood work and hospital storyline to reveal the real bio-mom.
